Fabricate Studios Student Forms

Welcome to Fabricate Studios. As a student, or parent of a student at Fabricate Studios, the following forms and waivers are required for each person attending a class, series of classes, workshops, and camps at our studio.

You can easily download each PDF form, print it and fill it out prior to arriving at the studio. We will have blank copies available at the studio as well.

For adults attending classes at Fabricate Studios, please complete, sign and submit our Adult Student Waiver.

For children attending classes, Fabricate Studios has a series of forms required. These forms are required for all first-time students as well as returning students. First time students, parents will need to complete the entire packet. Returning students will need to complete the forms as indicated by the instructor(s).
