Sewing, knitting, embroidery…crafting is serious business, ya’ll! And we use tools and equipment as part of teaching these crafty skillz. By making a purchase from Fabricate Studios you acknowledge that participation exposes the student to a possibility of personal injury. You, being fully aware that participation exposes you to a possible risk of personal injury, hereby release Fabricate Studios and its owners, officers, directors, employees, agents, licenses, subsidiaries, consultants, independent contractors and affiliates from any and all liability from property damage, personal injuries, or other claims arising from or in connection with your participation including claims that are known and unknown, foreseen and unforeseen, future or contingent. You acknowledge that you have read and fully understand this Injury Waiver and General Release Form. This agreement shall be binding on you, your spouse, your children, legal representatives, heirs, successors and assigns.

Additionally, Fabricate Studios (“Fabricate”) has put in place preventative measures to reduce the spread of COVID-19; however, Fabricate cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending Fabricate could increase your risk and your child(ren)’s risk of contracting COVID-19.

You acknowledge that you have read and fully understand this acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that your child(ren) and yourself may be exposed to or infected by COVID-19 by attending Fabricate and that such exposure or infection may result in personal injury, illness, permanent disability, and death. You understand that the risk of becoming exposed to or infected by COVID-19 at Fabricate may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Fabricate instructors, volunteers, and program participants and their families.

You voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to your child(ren) or yourself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that you or your child(ren) may experience or incur in connection with your child(ren)’s attendance at Fabricate (“Claims”). On your behalf, and on behalf of your children, you hereby release, covenant not to sue, discharge, and hold harmless Fabricate, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. You understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Fabricate, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Fabricate class, session, or program.
